REGISTRATION COSTS
"1/2 Registration Fee is Due at Registration Sign up"
Balane will be due no later than Oct 24th 2011
Players will be removed from ice if not paid by 10/24 or arrangements made
Registration Team will be @ rink during practice the week of 10/17 to collect remaining balance

$80 + USA Hockey Registration Fee - New Players that have never played hockey before (Borin from 2007 - 1993)

$80 + USA Hockey Registration Fee - Mites (Borin in 2007, 06, 05, 04, or 03)

$230 + USA Hockey Registration Fee - Squirt (2002 or 01), Peewee (2000 or 1999), Bantams (1998 or 1997)

$170 + USA Hockey Registration Fee - Midget (Split Season) (1996, 95, 94, 93)

Note: Squirt, Peewee, Bantam, Midget will need a home and away jersey that must be purchased through SRIHA. Cost is $60 for home & $60 for away jersey totaling $120.
If Returning players DO NOT have all completed paperwork and money turned in by August 21st they will be charded a late fee of $50. Make arrangments with Carol Ackely prior to August 21st if you cannot attend registration.
Rental Gear
Rental Gear for players born in or after 2003 is available on a first come first server at a fee of $50 that will be refunded when equipment is returned and is in good condition. Rental Gear does not include skates or stick. There is some rental gear for older kids but it is very limited.
Fundraising Responsibilities
We do not require door to door fundraising. Although all parents are REQUIRED to work the snackbar during games (most teams divided up it to one period per home game between all the families, so the amount of periods worked varies by team size), work public skate when their team is assigned, work their home tournament, and solicit raffle items for tournament. Salmon River Hockey is completely run by volunteers and this is why you are REQUIRED to work the snackbar, public skate, your home tournament, and solicit raffle items
Required Parent Education Meeting
At least one parent must attend the parent educatiton meeting prior to their children being allowed on the ice. This is a mandatory meeting by NYSAHA, if we do not have these meetings will not be in good standing. Meetings will be Sunday September 25th at 6 pm and Sunday October 2nd at 6 pm at the Ice Rink. Their will be a sign-up sheet at registration.
Player Registration Instruction
1. Register (each player) for USA Hockey at http://www.usahockeyregistration.com/
o Select Ice Players & Coach (Coaches no not register online at this time)
o Select 2011-2012
o Follow steps, print out confirmation and bring to registration
2. Fill out following forms per family. Forms located on top left of this page
o 2011-12 SRIHA Reg Form (bring to registration)
o 2011-12 Parents Code of Conduct (bring to registration)
o 2011-12 Fundrasing and Parent Commitment Form (bring to registration)
3. Fill out following forms for each player. Forms located on top left of this page
o 2011-12 Consent to Treat (bring to registration)
o 2011-12 Code of Conduct (bring to registration)
o 2011-12 Jersey Order Form, if needed (bring to registration)
Note: If you are unable to perform the following prior to registration we will have forms and computers available at registration. You will need to bring your credit/debit card to register for USA Hockey.
